The role:
This is an exciting opportunity to work in one of Ireland's largest non-profit organisations who provide services and support to people that live with Dementia and Alzheimer's Disease.
We are recruiting a Homecare Coordinator who will be responsible for coordinating the home care service across Tralee along with assessing the needs of clients and carers, assigning staff to clients based on their needs and reviewing these assignments regularly.
We are interested in receiving applications from applicants who have worked in health, social care and voluntary care backgrounds and who have relevant experience treating patients or residents who live with Dementia, Alzheimer's disease, intellectual disabilities and have a passion for working in community settings.

Essential Criteria:
- Must be eligible to work in Ireland (no visa sponsorship is provided for this role).
- 1-2 years' experience in a health/social care or voluntary background is essential for this role.
- Experience working with people with Dementia or intellectual disabilities is essential.
- HR & Financial management, Quality standards and risk management experience would be desirable.
- Good communication skills, be open and enthusiastic, have a positive attitude and lead by example.
- Need to be able to work on your own initiative as well as part of a team.
Key Duties:
- Oversee the day-to-day operations of both client and stakeholders.
- Conducting risk assessments in resident's homes and engaging with staff in relation to same.
- Have an understanding of HSE and HIQA governance.
- Complete care plans for clients care and update where necessary.
- Adapt a direct approach to clients in reference to competency skills assessments with all Healthcare staff.
- Apply a person-centered model of care to all clients.
- Responsibility for own continuous professional development
- Support Staff, through effective communication, employee appraisal process and identifying and development of talent.
- Complete ongoing reviews of the services and clients and report back to the operations manager.
