We're recruiting on behalf of a leading homelessness and social inclusion organisation seeking a dedicated Complex Needs Case Worker to support individuals experiencing multiple and interconnected challenges such as homelessness, addiction, poor mental health, trauma, offending behaviour and social exclusion.

This is a highly rewarding community-based role where you will manage a small caseload of individuals, build trusted relationships, coordinate multi-agency support, and help people achieve greater stability, independence and improved wellbeing.

About the Role

Working within a multidisciplinary team, you will provide intensive, person-centred support to individuals with complex needs. Using a housing-led and trauma-informed approach, you will coordinate support plans and act as the key point of contact for service users and partner agencies.

Key responsibilities include:

• Managing a caseload of approximately 7-10 individuals with complex support needs
• Developing and coordinating personalised support plans
• Building strong, trusting relationships with service users
• Supporting individuals to access accommodation, healthcare and community services
• Working closely with statutory, voluntary and community organisations
• Organising and attending multi-agency meetings and case reviews
• Monitoring progress, maintaining accurate records and reporting outcomes
• Supporting individuals through crisis situations and periods of disengagement
• Promoting recovery, resilience, independence and positive lifestyle choices

What We're Looking For

Essential Criteria:

✔ Level 3 qualification in Health & Social Care or a related field

✔ Minimum 2 years' paid experience supporting vulnerable adults or complex client groups

✔ Experience of case management, support planning or key working

✔ Strong communication and relationship-building skills

✔ Full UK driving licence and access to a vehicle insured for business use

✔ Willingness to travel throughout the local area
