Location: Ennis, Co. Clare

Hours: Monday to Friday, 30 hours per week

Pay Rate / Salary: Salary DOE

About the role:

The Home Care & Daycare at Home Coordinator is responsible for providing best-practice dementia-specific care to clients in the community and their homes. The role coordinates person-centred care and resources, supervises Care Workers, and supports the delivery of services for people living with dementia and their family carers. The Home Care & Daycare at Home Coordinator will suit someone with experience in caring for people with dementia and managing people or services.

Key Duties:

  • Supervise, coordinate and support Care Workers, including recruitment, induction, training, and performance reviews.
  • Manage staff rosters, records, pay sheets, travelling expenses and administration.
  • Assess and manage the needs of potential and existing clients and develop appropriate care plans.
  • Review clients regularly and consult with family carers, health and social care professionals and other service providers.
  • Identify gaps in care provision, manage waiting lists and address health and safety concerns through risk assessments.
  • Provide support and education to family carers, including phone support and information on issues arising in client care.
  • Maintain accurate records, statistics and reports and ensure compliance with relevant policies and service requirements.
  • Network with service providers and referral sources and promote local services.

Essential Criteria:

  • 2-3 years' experience in the health/social care or voluntary sector.
  • Previous experience in a caring environment, preferably caring for people with dementia, older people, or people with learning disabilities.
  • Experience or understanding of managing people or a service.
  • Experience collaborating with people with complex needs in different situations.
  • Understanding of relevant legislation, the Irish health system, and structures, with strong MS Office skills.
  • Full clean driving licence
