Outreach Case Worker - Housing First

Derry/Londonderry | Permanent | 27 Hours per Week

Salary: £15.41 per hour

Our client is seeking an experienced and compassionate Outreach Case Worker to support individuals experiencing homelessness and complex needs within a Housing First service in Derry/Londonderry.

Working as part of a supportive team, you will provide person-centred, community-based support, helping service users sustain tenancies, improve wellbeing, access services, and achieve greater independence.

Key Responsibilities

• Deliver tailored support and key working sessions.
• Develop and review support and risk management plans.
• Support service users to access housing, health, and community services.
• Advocate on behalf of service users to achieve positive outcomes.
• Complete outreach visits within the local community.
• Maintain accurate records, reports, and case notes.
• Contribute to a safe, professional, and supportive service.

Essential Criteria

Either:

• Level 3 qualification in Health & Social Care (or equivalent) plus 2 years' relevant paid experience

Or:

• 3 years' relevant paid experience and 3 Level 2 qualifications including Maths and English

And:

• Full UK driving licence
• Access to a vehicle insured for business use
• Ability to work a rota including varied shifts
